Has anyone on the list ever set up an authenticating squid proxy which would
then allow you to see what sites each individual user was hitting?

Yes.

Once you've got authentication setup, squid will log the authenticated
username with each request. You can then use something like SRG, or
SARG, or Calamari, or any number of other tools to mine the logfiles for
review. Or, if you just need an ad-hoc report you could use grep and awk ;)
